Advocate Health in Park Ridge, IL is seeking a seasoned Advanced Practice Nurse to deliver high-quality, age-appropriate care within an interdisciplinary team. You will assess patients, develop and adjust treatment plans, and coordinate with physicians, nurses, and support staff to ensure safe, effective care.

Responsibilities include promoting health, preventing disease, documenting meticulously in the EMR, and participating in quality initiatives.

Qualifications

Active RN and APRN licenses in state(s) of practice.
Active national board certification in area of clinical practice.
Active DEA registration before hire.
Illinois practice requires Illinois Controlled Substance License before hire when applicable.
Master’s Degree in Nursing.
Experience in Primary Care, Pediatrics or School Based Health Center preferred.

Responsibilities

Assesses, diagnoses, and determines treatment for age, acuity and clinical condition.
Manages conditions using evidence-based care, cost effectiveness, and risk/benefit assessment.
Provides health promotion, disease prevention, and patient/family education.
Works within an interdisciplinary team and seeks referrals as appropriate.
Maintains accurate EMR documentation and compliant coding/billing.
Facilitates coordinated care and clear communication among health team and community agencies.
Performs procedures per privileges where applicable.
Participates in quality, safety, and peer review initiatives and meetings.
Educates and onboarding of new team members and other healthcare professionals.
Maintains productivity, access, face-to-face time, and quality metrics.
