Job summary

Carewell Supportive Care, a member of Empath Health, seeks a Palliative Care APRN to provide comprehensive, patient-centered care across hospital, community, and office settings in the Ocala area.

The role emphasizes symptom management, advanced care planning, and goals of care in collaboration with the Medical Director and interdisciplinary team. Standard hours are Mon–Fri 8:00–5:00 with rotating Saturdays.

Qualifications

• Master’s degree in nursing or related field required.
• Florida Advanced Registered Nurse Practitioner license with board certification.
• Current DEA license and CPR certification required.
• Two years of nursing experience; APRN experience in adult/gerontology/oncology preferred.
• NPI provider and CMS billing ability preferred.

Responsibilities

• Provide consults across hospitals and community settings in Ocala area.
• Manage symptoms, provide advanced care planning, and goals of care.
• Develop and evaluate individualized care plans in collaboration with medical team.
• Coordinate care across hospital, clinic, and home-based visits.
• Collaborate with physician and interdisciplinary team; provide hospice cross-training.

Carewell Supportive Care, a member of Empath Health is seeking a Palliative Care APRN. The APRN in the Community-Based Palliative Care Program provides comprehensive, patient‑centered care in collaboration with the Medical Director and interdisciplinary team. This role focuses on assessing complex needs, managing symptoms, and supporting patients and families through serious illness. The APRN develops and evaluates individualized care plans, promotes self‑care, and ensures alignment with patient and family goals. Through clinical expertise and compassionate communication, the APRN helps deliver high‑quality, holistic palliative care across hospital, community, and office settings.Standard hours are Monday–Friday, 8:00 AM–5:00 PM, with participation in a rotating Saturday schedule that replaces a weekday.Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care.
