Job summary

Apple Rehab West Haven is seeking a compassionate CNA for per diem appointment shifts at our 308 Savin Avenue facility in West Haven, CT. Join a family‑oriented team delivering high‑quality resident care and flexible per diem opportunities.

Starting pay is competitive, with per diem shifts designed to fit your schedule. You’ll assist residents with daily needs and document care, while upholding our commitment to dignity and respect in a supportive environment.

Qualifications

• Must hold current Certified Nursing Assistant (CNA) certification in good standing.
• No history of patient/resident abuse as per State Registry.
• Proficient in reading, writing, and following oral and written instructions.
• Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks.
• Team player with good cooperation and interpersonal skills.
• Demonstrates patience, tact, enthusiasm, and a positive attitude towards residents and their support networks.

Responsibilities

• Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use.
• Positioning in and out of bed and documenting personal care given according to facility policies.
• Provides nursing functions as directed including perineal care, making beds, turning residents, and recording vital signs.
• Observes and reports changes in residents’ condition to supervisor, including skin breakdowns.
• Reports immediately all accidents or injuries and records all necessary charting entries.
• Answers all call lights regardless of assignment and responds to emergencies calmly and efficiently.

Appointment Companion - CNA Certified Nursing Assistant

Description:

Looking for CNA's who are interested in picking up resident appointment shifts per diem. CNA's are welcome to pickup floor shifts as well.

Starting Pay:

Competitive rates starting at $21.22/hour

Available Shifts:

• Per Diem Appointment Companion

Responsibilities:

Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use, positioning in and out of bed. C.N.A. Certified Nurse’s Assistant documents personal care given according to facility policies. Provides nursing functions as directed including perineal care, making beds, turning residents, measures and records temperature, weight, pulse and respiration. C.N.A. Certified Nurse’s Assistant observes and reports to supervisor changes in residents’ condition, including any skin breakdowns. Reports immediately all accidents or injuries. Records all necessary charting entries. Answers all call lights regardless of assignment. Responds to emergency situations calmly and efficiently.

Qualifications:
