Job summary

Valley Health System in Paramus, NJ, seeks an Advanced Practice Provider to support the Structural/Interventional program. Working with a multidisciplinary team, you will participate in pre- and post-procedural care, education, coordination of care, and quality improvement efforts to ensure optimal patient outcomes.

You will collaborate with supervising physicians to develop and implement comprehensive care plans, educate patients and families, and promote wellness.

Qualifications

  • Graduate of an accredited Physician Assistant program.
  • Current Nurse Practitioner program graduate or PA certification required.
  • New Jersey license to practice is required upon employment and BLS certification.

Job description

Position Summary

In a collaborative and multidisciplinary environment, contributes professional and clinical expertise to assigned patient populations to ensure the delivery of high-quality care. The Advanced Practice Provider serving the Structural/Interventional program will be involved in the patient’s longitudinal care including pre and post procedural care, education, coordination of care, procedural assistance, follow-up, and quality improvement initiatives to ensure optimal patient outcomes and program success. Duties are performed in accordance with the NJ Board of Nursing guidelines for APN or BOME guidelines for Physician Assistant 1.

Expert Clinical Practice

In collaboration with collaborating/supervising physician, assists in all aspects of inpatient and outpatient care for a select patient population. Maintains expertise in area of practice and applies advanced clinical skills to care for that select patient population. Incorporates advances in technology and procedures into skill set, remains current in practice of palliative care. Assist in the evaluation of outcomes and adjustment of practices to ensure optimum delivery of care in a cost-effective manner. Facilitate the coordination and the communication of patient care needs across the health care continuum.

Expert Guidance and Coaching

In collaboration with the collaborating/supervising physician, implements a therapeutic comprehensive plan of care for patients and families monitoring all aspects of the therapeutic regimen. Educates patients and families in prescribed treatment plan of care. Educates and guides families to promote wellness and assume responsibility for participation in the management of their own health care. Educates patients and families to facilitate understanding of the patient's medical condition and proposed course of therapeutic intervention. Serves as a resource person for education of health care professionals and the community-at-large to promote health maintenance and disease prevention.

Consultation/Collaboration

Serves as a clinical resource and expert throughout the institution in area of specialty; maintains knowledge of system operations to expedite unit needs (e.g. who to contact, best methodology to accomplish desired result). Responds to consultative requests consistent with the directions of the supervising physician and provides recommendations for clinical/educational/psychosocial interventions as necessary. Collaborate with multidisciplinary care team as needed to optimize quality and delivery of care.

Research

In collaboration with multidisciplinary staff, supports research initiatives of the organization through education, practice, and leadership. Critically evaluates existing practice based on current research findings and integrates changes into practice. Incorporate research findings and quality improvement strategies into clinical practice to improve patient outcomes. Participates in research projects as appropriate, maintains documentation and databases to support projects.

Leadership

Act as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care. Foster communication between members of multidisciplinary care team. Identifies the need for new programs, assesses potential outcomes and cost/benefits, and makes recommendations for further development, implementation, and evaluation. Works with other departments to establish new programs for patient care delivery, assessment, and evaluation. Monitors practice patterns for trends in outcomes and resource utilization for patient populations in area of practice.

Experience Minimum

Minimum of 1-3 years of clinical experience, primarily in area of specialty.

Special Skills/Qualifications

Graduate of an accredited Physician Assistant program as determined by the Committee on Allied Health. Education and Accreditation or a current Registered Professional Nurse and Graduate of an accredited Nurse Practitioner Program/master’s in nursing. Current Certification from the National Commission on the Certification of Physician Assistants (NCCPA) or for Advanced Practice Nurses a current Board Certification from the American Nurses Credentialing Center (ANCC) or from the American Academy of Nurse Practitioners (AANP) Hold a valid, current New Jersey license. Obtain a New Jersey CDS and federal DEA certificate upon employment if not already covered. Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving. Participation in pertinent professional and/or specialty organizations preferred. BLS certification.

Salary

Salary Minimum new grad base salary $143,200 K. Salary commensurate with years of experience as an NP/PA. Ability to earn up to 5% quality incentive.
