The position requires coordinating referrals for mental health assessments, supporting ongoing counseling, and keeping discharge plans coordinated with all parties involved.

Qualifications

• Master's degree in social work required.
• Active LSW license in the state of Ohio.
• Experience working with clients with serious mental illness (SPMI).

Responsibilities

• Provide collaborative oversight of the AOT program and coordinate care between hospitals, agency staff, and probate court.
• Coordinate mental health assessments for referred clients and provide ongoing clinical counseling.
• Educate hospital systems on AOT processes and workflows to access internal services.
• Ensure ROIs are completed and schedule follow-up appointments within seven days of hospital discharge.
• Coordinate with probate court, psychiatry staff, and case managers to schedule recommitment hearings when needed.
