Job summary

Baystate Health in Massachusetts seeks a registered nurse for the Triage RN, Ambulatory Nurse – Community Health Adult Medicine role. This position supports outpatient primary care in the Community Health Centers, provides triage and plan-of-care development, and promotes wellness across diverse patient groups.

Licensure in Massachusetts and BLS CPR are required, with a minimum of 2 years of nursing experience.

Qualifications

• Massachusetts RN license and BLS CPR certification required.
• Experience in outpatient, primary care or health center setting preferred.
• Excellent verbal and written communication and interpersonal skills.

Responsibilities

• Assess, develop and implement plan of care for complex patients including medical and/or psychosocial disorders.
• Provide independent triage and patient care using assessment skills, point-of-care testing and QA checks.
• Establish plan of care, set priorities and implement with patient/family and team input.
• Build knowledge to include more diagnoses and complicated medical patients.
• Triages and coordinates patient care ensuring access to care and patient success.
• Promote health and wellness for individuals and groups of patients.
• Demonstrate proactive approach to acute and chronic disease management.
• Advocate for patient rights, values and beliefs; develop patient trust and engage family in care.
