Job summary

Community Health Programs (CHP) is seeking an experienced Registered Nurse to join our ambulatory practice in the Berkshires, MA. The RN will triage by phone or walk-in, support the Practice Manager, and assist in patient care under CHP physicians, delivering compassionate, patient-centered care.

Responsibilities include patient education, scheduling, immunizations, coordinating referrals, and supporting colleagues in a fast-paced environment while adhering to CHP policies and compliance

Qualifications

• RN or BSN required.
• Current BLS certification.
• Minimum 3 years’ ambulatory experience preferred.
• Electronic medical records experience required; Athena is a plus.
• Massachusetts RN licensure.

Responsibilities

• Facilitates comprehensive care for all patients.
• Provide patient education, including pregnancy visits and post-partum.
• Schedule appointments and coordinate referrals.
• Screen and triage patients by phone or in person; support same-day visits.
• Assist physicians and supervise medical assistants as needed.
• Administer immunizations and manage prescriptions.
• Maintain patient flow and assist with rooming.
