Sentara Health Plans in Norfolk, VA is hiring a Care Coordinator to perform casework and provide case management to patients, families, and caregivers. You will develop and monitor multidisciplinary services, assisting adults with medical and environmental challenges, including end-of-life care where appropriate.

The role requires a Health & Human Services degree (or related field) from an accredited program, 1 year in Long Term Care, and 3 years in care coordination preferred.

Qualifications

• Bachelor's or higher degree in Health and Human Services or related field from an accredited program.
• 1 year in Long Term Care required; 3 years in care coordination preferred.
• Experience in adult patient care, family support, and multidisciplinary collaboration preferred.

Responsibilities

• The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
• Develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate.
• Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
