Qualifications

Responsibilities

• Conduct biopsychosocial assessments to evaluate patient needs and barriers.
• Develop individualized, patient-centered care plans.

• Document and bill for services in compliance with requirements.
• Identify crises and provide crisis intervention and support.
• Monitor progress and evaluate outcomes of care plans.
• Advocate for patient needs within the healthcare system.
