Job summary

UNC Health in North Carolina is expanding social work services within the Wound Healing, Podiatry, and Vascular programs to improve access, care coordination, and psychosocial support for patients with complex medical needs.

As a Social Worker – Ambulatory II, you will conduct assessments, develop person-centered care plans, facilitate resources, advocate for patients, and monitor progress as part of an interdisciplinary team. This role is onsite with standard hours and a 40-hour workweek.

Qualifications

• MSW from CSWE-accredited program.
• LCSW licensure in North Carolina required; LCAS preferred.
• At least 3 years of healthcare social work experience.

Responsibilities

• Conduct biopsychosocial assessments to evaluate patient needs and barriers.
• Develop individualized care plans with the multidisciplinary team.
• Facilitate connections to community resources and referrals.
• Coordinate care transitions to optimize outcomes and reduce hospitalizations.
• Document services and ensure billing compliance.
• Provide crisis intervention and brief motivational support as needed.
• Monitor progress and update care plans with the team.
• Advocate for patient access to treatment and resources.
• Engage in active listening and culturally sensitive care.
• Lead team meetings and provide education on standard workflows.
• Serve as a consultative resource for staff.
