Qualifications

• Master's Degree in Social Work required.

Responsibilities

• Assessments: Conduct biopsychosocial assessments to evaluate patient needs and barriers.
• Patient-Centered Care Plans: Develop individualized plans with medical and social support.
• Resource Facilitation: Connect patients with community resources and services.
• Documentation and Billing: Record and bill for services per requirements.
• Crises Management: Identify crises and provide intervention and planning.
• Progress Monitoring & Evaluation: Track care plan progress and adjust as needed.
• Advocacy: Ensure access to treatment and effective communication with the care team.
• Active Listening: Listen to patients' concerns and provide empathetic support.
• Respect for Individuality: Honor diverse backgrounds in care planning.
• Building Trust: Maintain confidentiality and foster therapeutic relationships.
• Meetings & Quality Improvement: Participate in team problem solving and initiatives.
