Job summary

VCU Health is seeking a Care Coordinator Team Lead to provide clinical leadership and mentoring to RN Care Coordinators and Social Workers on the outpatient transplant service lines at the Hume Lee Transplant Center. The role supports daily clinical needs, complex case review, and cross-functional collaboration with physicians and front-line staff.

The Team Lead participates in maintaining best practices across organ programs, improving communication, and supporting the Social Work Manager.

Qualifications

• Minimum of five years of clinical experience.
• Mentoring or clinical leadership experience preferred.
• RN licensure in Virginia or eligible; LCSW licensure in Virginia preferred.

Responsibilities

• Provide clinical leadership and mentoring to RN Care Coordinators and Social Workers.
• Facilitate patient care, problem-solving, and resource stewardship on the designated team.
• Contribute to clinical, quality, and financial performance of the Department.
• Proactively manage daily clinical needs and escalation of complex cases.
• Coordinate with interdisciplinary teams and transplant leadership to ensure coverage and continuity.
