The Team Lead will be available to provide counsel and support for daily clinical patient needs, consult regarding complex cases, maintain and support best practices across all individual organ programs, and improve communication with front line staff and their interdisciplinary teams. This Team Lead will support the Social Work Manager by escalating issues as needed, improve retention and commitment to the team, and coordinate daily coverage needs. The position will work closely with both front line staff, Care Coordination and Hume Lee Transplant leadership across the service line.A successful Team Lead is one that enjoys interdisciplinary collaboration, problem solving and demonstrates excellent communication skills.Work hours are normal business hours, Mon-Fri, major holidays off with PTO, no on call or weekend expectation.
