Job summary

Rush University Medical Center in Chicago is seeking an Ambulatory Social Work Care Manager I to provide patient-centered care coordination within the MHN Primary Care Case Management team.

The role requires an IL-licensed social worker (LSW or licensed) with MSW, strong communication, crisis management, and data-informed care planning. The position offers full-time hours, on-site schedule 8:30 AM–5:00 PM, and a competitive hourly pay range.

Qualifications

• Master’s degree in Social Work required.
• Current IL license as a Social Worker, LSW or proof of pending license within 90 days of hire.
• Experience with psychosocial issues, crisis management, conflict resolution, person-centered planning, and care transitions.
• Willingness to co-locate in the outpatient clinic when needed.
• Proficient in Microsoft Office Suite.
• Ability to perform tasks independently, prioritize workload, problem-solve, and analyze data.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Identifies patients needing outreach using MHN portal and EMR reports based on inter-professional team referrals, clinic referrals, ER, and discharges.
• Assesses physiological, psychosocial, financial, cultural, and familial situations and coordinates psychosocial services.
• Develops psychosocial care plans and acts as a change agent for complex cases.
• Aligns care plans with available resources and promotes cost-effective, quality care.
• Participates in program development, implementation, and evaluation within RUMG clinics.
• Provides crisis intervention when needed.
• Delivers educational materials to patients/families in accessible formats and languages.
• Monitors care management outcomes using defined quality measures.
• Maintains a proactive, patient-centered approach to meet department/institution goals.
• Documents per regulatory guidelines and coordinates with teams on progress and discharge planning.
• Organizes clinical supervision for social work staff and manages education needs.
