Job summary

Qualifications

• Master's Degree in Social Work from an accredited university.
• Current license in Illinois as a Social Worker (LSW) or proof of pending license within 90 days of hire.
• Ability to manage complex cases within social work practice and work as a team resource.
• Experience with health care providers across neonate to geriatric populations and care management.
• Knowledge of psychosocial issues, crisis management, and person-centered planning.

Responsibilities

• Provide psychosocial assessments and coordinate transition plans.
• Educate patients on levels of care, entitlements, and community resources.
• Coordinate discharge planning and continuity of care.
• Lead and participate in cross-disciplinary team communications.
• Document all activities in the medical record and assist with referrals.
