Job summary

UW Health is seeking an Ambulatory Social Work Specialist to triage ambulatory clinic social service cases and transportation needs. You will delegate cases, document reports, and coordinate care with UW Health staff while addressing community referrals.

Apply Trauma-Informed principles in every interaction to support patient wellbeing. The role requires a Bachelor's degree in a behavioral science field and at least 1 year of related experience, preferably with a medical setting.

Qualifications

• Bachelor's degree in psychology, social work, counseling, or behavioral science related degree required.
• 1 year related experience required.
• Experience working with a diverse population base with complex social/emotional needs required.
• Previous medical setting experience preferred.

Responsibilities

• Serve as centralized triage for ambulatory clinic social service case and medical appointment transportation needs.
• Delegate and distribute cases, document reports, and manage less complex cases including care coordination and community referrals.
• Apply Trauma Informed principles with all interactions.
• Complete patient needs assessments and analyze chart documentation to address social determinants of health barriers affecting adherence and wellbeing.
