Indiana University Health System in Muncie, Indiana is seeking an experienced RN to join a collaborative ambulatory care team. The role focuses on care coordination, chronic disease management, and patient education across FMOB and Internal Medicine under one roof.

The position supports a busy practice with a schedule from 8:00 a.m. to 4:30 p.m., with no travel requirements, offering an opportunity to impact patient outcomes within a supportive team environment.

Qualificações

• Requires an Associates of Nursing (ASN); BSN preferred.
• Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
• Requires Basic Life Support certification through the AHA.
• Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
• May require ability to assess patients without face-to-face interaction.
• Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.

Responsabilidades

• Provide care coordination for FMOB and Internal Medicine patients.
• Manage chronic diseases and patient education.
• Coordinate medications and referrals.
• Close care gaps and follow-up after hospitalization.
• Support a team of providers and clinical staff to drive continuity of care.
