Supporting a unique practice that combines Family Medicine Obstetrics (FMOB) and Internal Medicine under one roof, you will play a key role in providing care coordination, chronic disease management, patient education, medication management, referral coordination, care gap closure, and post-hospitalization follow-up for patients across the lifespan, including infants, obstetric patients, families, and adults. Working alongside 10 providers, 2 dedicated triage nurses, and 11 clinical support staff, you will help drive continuity of care and positive patient outcomes in a busy and rewarding ambulatory setting. This full-time day shift position offers a consistent schedule of 8:00 a.m. to 4:30 p.m., no travel requirements, and the opportunity to make a meaningful impact within a highly supportive team environment.
