Qualifications

• Current RN license in state of practice is required.
• BLS for healthcare providers is required.
• Practice-specific certifications (ACLS, PALS, etc.) as needed.
• Ability to communicate effectively verbally and in writing.
• If RN with under 12 months, must obtain BSN within four years.

Responsibilities

• Assess physical, social and psychological status of patients.
• Develop individualized plan of care using assessment data.
• Coordinate care with interdisciplinary team and family.
• Direct and coordinate the care team to implement the plan.
• Evaluate outcomes and revise the plan as needed.
