Qualifications

• Current RN license in state of practice.
• BSN preferred by the clinical setting.
• Ability to communicate effectively both verbally and in writing.

Responsibilities

• Assess patients’ physical, social and psychological status.
• Plan and develop individualized care plans with patient and family.
• Coordinate with interdisciplinary team to implement care.
• Direct the care team and delegate tasks per plan of care.
• Evaluate patient responses and adjust plans as needed.
