Job summary

SEARHC is seeking an Ambulatory Registered Nurse Case Manager in Juneau, Alaska for a full-time position in an outpatient clinic setting. You will use the nursing process to assess, plan, implement, and evaluate care, while coordinating with a multidisciplinary team to promote safe, effective, and culturally responsive services.

Responsibilities include medication administration, patient education, documentation, and care coordination.

Qualifications

• Associate degree in Nursing from an accredited program.
• Current, unrestricted RN license in Alaska or another US state.
• BLS certification required.
• Case Management certification preferred.
• Clinical competency within 3 months and every 3 years.
• High Risk Competency within 3 months and yearly.

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Administer medications and treatments per orders and policy.
• Provide patient and family education and discharge planning.
• Document assessments and outcomes in the electronic health record.
• Coordinate care with providers and interdisciplinary teams.
• Escalate care as needed and ensure safety and quality.
