Job summary

The Marion VA Health Care System is seeking a full-time Registered Nurse to serve as the Care Coordination RN on the Referral Coordination Team within The Group Practice Manager under the Chief of Staff services located in Marion - IL.

duties include communicating with Veterans about their health care options, acting as liaison between referring providers and specialty care, and overseeing outpatient clinic care coordination and patient flow management to ensure timely processing of referrals.

Qualifications

• Must be licensed as a registered nurse in the United States.
• Graduation from an accredited school of nursing and current RN licensure.

Responsibilities

• Coordinate outpatient referrals and patient flow within the Referral Coordination Team.
• Serve as liaison between referring providers and specialty care services.
• Assist with triage, care coordination, and review of medical necessity for referrals.
• Provide clinical expertise to multidisciplinary staff and support patient care coordination.
