Prima Medical Foundation dba MarinHealth Medical Network seeks a Population Health Program Manager to guide ambulatory initiatives, improve preventive and chronic care, and optimize value-based performance.

The role partners with payors, UCSF population health teams, and clinical leadership to execute strategies that boost quality, access, and patient engagement. Requires strong analytic and change-management skills and a track record in ambulatory settings.

Qualifications

• Bachelor's degree required.
• Master's degree preferred.
• Minimum 5 years of progressive experience in ambulatory healthcare operations, population health, care management, quality improvement, or value-based care.

Responsabilités

• Lead organization-wide population health initiatives and close care gaps across preventive, chronic disease, and wellness measures.
• Develop workflows to improve performance on HEDIS, CMS Star Ratings, quality metrics and incentives.
• Create dashboards and analytics tools to monitor care gaps and outcomes.
• Collaborate with clinicians, payors, and leadership to improve access and outcomes.
• Lead education and change-management initiatives for ambulatory teams.
• Support payor partnerships and performance improvement projects using Lean or DMAIC.
