Job summary

GBMC HealthCare is seeking a Care Coordinator for Population Health to strengthen quality performance through data and reporting. The role focuses on monitoring gaps in care, coordinating outreach, and linking patients to services in an ambulatory setting, with collaboration across the care team at Towson.

The candidate will work with physicians, nurses, and behavioral health staff, ensuring smooth operation and patient-centered care across the care continuum.

Qualifications

• High School graduate or higher.
• 3 years medical office experience navigating the healthcare system.
• Knowledge of medical and insurance terminology.
• Strong communication skills (oral and written).
• Proficiency with EMR, email, internet, spreadsheets and databases.
• Ability to analyze and interpret reports.

Responsibilities

• Promote population health management and quality performance via data and reporting.
• Oversee care coordination activities for patients in need.
• Link patients to health care services, community resources, and social supports.
• Generate and review patient reports from the EHR to identify gaps in care.
• Collaborate with the Practice Manager, RN Care Manager and clinical team.
• Provide back-up support to other Care Coordinators when needed.
