Job summary

ChenMed in the United States is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics under a collaborative practice agreement. You will work with physicians, nurses, and care teams to deliver evidence-based, cost-effective care.

A PharmD and active license are required; residency or ambulatory care certification is preferred. You should be able to travel locally up to 50% and be patient-focused with strong communication and clinical decision skills.

Qualifications

• PharmD degree required and active license in good standing.
• Ambulatory care residency or certification preferred (e.g., BCACP).
• Minimum 2 years clinical experience in primary or ambulatory care.
• Immunization and medication therapy management credentials are a plus.

Responsibilities

• Collaborates with providers to optimize therapies within a protocol-driven framework.
• Conducts comprehensive medication reviews addressing safety, cost, and adherence.
• Manages chronic diseases (diabetes, hypertension, anticoagulation, asthma).
• Facilitates transitions of care and prevents readmissions through reconciliation.
• Educates patients and engages them to improve adherence.
• Orders/interprets labs to monitor safety and efficacy.
• Works with physicians to implement care protocols and document updates.
• Performs other duties as assigned by management.
