Job summary

UNC Health Care's UNC Hospitals Department of Pharmacy is seeking a per diem Clinical Pharmacist Practitioner to support ambulatory care clinics in the Triangle West/Chapel Hill region. This part-time role requires a consistent one day per week commitment, with potential for additional hours to meet clinic needs.

The CPP will function as an Advanced Practice Provider on a multidisciplinary team, delivering direct patient care, pharmacotherapy management, and collaboration with physicians,

Qualifications

• Bachelor's Degree in Pharmacy is required.
• Must be a registered pharmacist in NC and licensed CPP per UNC HealthCare privileging.
• Completion of a PGY2 residency in Ambulatory Care or Pharmacotherapy, or equivalent.
• Three to five years ambulatory care experience strongly preferred.
• Board Certification (BCACP or BCPS) preferred.

Responsibilities

• Build a clinical practice that collaborates with physicians to care for ambulatory patients.
• Provide longitudinal dosing and monitoring of regimens; manage transitions of care.
• Identify and enroll eligible patients in specialty pharmacy services and support distribution.
• Educate patients and families about treatment plans and medication management.
• Educate UNC Eshelman School of Pharmacy students and residents; serve on committees; design projects.
• Lead quality improvement initiatives and actively participate in program development.
