Job summary

Responsibilities

• Build a clinical practice in ambulatory clinics and collaborate with physicians to determine appropriate treatment plans.
• Provide education to patients, families and staff on treatment plans and medication management.
• Mentor pharmacy students and residents and participate in experiential rotations.
• Educate staff on guideline updates and practice advancements; participate in quality improvement initiatives.
• Identify and enroll eligible patients in specialty pharmacy services and coordinate with related programs.
