Job summary

UNC Medical Center in Durham, NC, seeks a per diem Clinical Pharmacist Practitioner to support ambulatory clinics in the Triangle West/Chapel Hill region. The CPP will function as an Advanced Practice Provider on a multidisciplinary team, delivering direct patient care and clinical support to physicians, nurses, and other staff.

Qualifications

  • Bachelor’s Degree in Pharmacy.
  • Registered Pharmacist and licensed CPP in NC prior to start date.
  • PGY2 residency in Ambulatory Care or Pharmacotherapy, or equivalent clinical experience.
  • Three to five years ambulatory care experience preferred.
  • Board Certification (BCACP or BCPS) preferred.

Responsibilities

  • Build a clinical practice with physicians and providers to care for clinic patients and educate on guideline updates.
  • Identify and enroll patients in specialty pharmacy services and coordinate follow-up.
  • Educate students and residents, participate in rotations, and conduct quality improvement projects.
  • Provide instruction to staff, develop programs, and lead community education initiatives.
  • Identify opportunities for new clinical programs and services to improve patient care.

UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated per diem Clinical Pharmacist Practitioner (CPP) to support ambulatory care clinics within the Triangle West/Chapel Hill region of UNC Hospitals.

This part‑time position is expected to be a consistent one day per week commitment, with the potential for additional time to support clinic needs, coverage gaps, or service expansion.

The successful Clinical Pharmacist Practitioner will play an integral role in managing patients cared for in ambulatory primary care and/or specialty clinic settings. The CPP will function as an Advanced Practice Provider (APP) on a multidisciplinary health care team and provide direct patient care as well as clinical support to physicians, nurses, and other ancillary staff. Ambulatory pharmacy services may include pharmacotherapy visits for chronic disease management, medication optimization, transitions of care support, and collaborative drug therapy management under approved privileges.

Within established ambulatory clinics, the CPP will collaborate closely with supervising physicians, clinic staff, and the Department of Pharmacy to provide direct patient care and medication management services. As clinic needs and available budgeted time allow, the CPP may also contribute to select quality improvement initiatives, workflow optimization efforts, or practice advancement activities relevant to the assigned clinical area.

The successful candidate may have opportunities to engage in educational mentorship or experiential precepting for pharmacy students and postgraduate pharmacy residents, dependent on clinic assignment, coverage day, and available funded time.

Responsibilities
  1. Build a clinical practice that collaborates with physicians and providers to care for clinic patients. Work closely with clinic physicians and providers to determine appropriate treatment plans, provide longitudinal dosing and monitoring of appropriate regimens, align daily practice with medication management goals set forth by the institution or pharmacy director, and educate patients, families, and staff on guideline updates, adverse effects, and transitions of care.
  2. Identify and enroll eligible patients in specialty pharmacy services such as UNC SSC Pharmacy, coordinate onboarding and follow‑up phone calls to support Pharmacy Adherence Centered Therapy, and collaborate with UNC SSC Pharmacy and the UNC Medication Assistance Program.
  3. Educate Eshelman School of Pharmacy students and UNC Medical Center pharmacy residents on experiential rotations, clinic staff on guideline updates, and serve as a standing committee member on selected groups of committees. Design and implement projects, conduct research, and prepare and submit manuscripts for publication.
  4. Provide instruction to physicians, nurses, ancillary staff, residents, and/or students through precepting, shadowing, lectures, in‑services and continuing education. Develop programs and lectures and participate in community outreach programs, while leading quality improvement initiatives.
  5. Engage in continuous quality improvement by identifying and implementing processes for improvement, consulting with management in assessment, problem‑solving, decision‑making, and evaluating clinical/system issues and models of care. Develop new clinical programs or services within the clinic/department to improve patient care.
Qualifications

Education Requirements: Bachelor’s Degree in Pharmacy.

Licensure / Certification Requirements: CPP must be a registered Pharmacist and licensed CPP in the state of NC. Credentialing and privileging must be completed through the appropriate UNC HCS accrediting body as an APP prior to start date.

Professional Experience Requirements:

  • Completion of a PGY2 residency in Ambulatory Care or Pharmacotherapy, or equivalent clinical experience.
  • Three to five years of ambulatory care practice experience strongly preferred.
  • Board Certification (e.g., BCACP, BCPS) preferred.
