Qualifications

• Bachelor's Degree in Pharmacy is required.
• Must be a registered pharmacist in NC and licensed CPP per UNC HealthCare privileging.
• Completion of a PGY2 residency in Ambulatory Care or Pharmacotherapy, or equivalent.
• Three to five years ambulatory care experience strongly preferred.
• Board Certification (BCACP or BCPS) preferred.

UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated per diem Clinical Pharmacist Practitioner (CPP) to support ambulatory care clinics in the Triangle West/Chapel Hill region of UNC Hospitals. This part‑time position requires a consistent one day per week commitment, with the potential for additional time to support clinic needs, coverage gaps, or service expansion.

The successful Clinical Pharmacist Practitioner (CPP) will play an integral role in managing patients cared for in ambulatory primary care and/or specialty clinic settings at UNC Health. The CPP will function as an Advanced Practice Provider (APP) on a multidisciplinary health‑care team, providing direct patient care as well as clinical support to physicians, nurses, and other ancillary staff. Ambulatory pharmacy services may include pharmacotherapy visits for chronic disease management, medication optimization, transitions of care support, and collaborative drug therapy management under approved privileges.

Within established ambulatory clinics, the CPP will collaborate closely with supervising physicians, clinic staff, and the Department of Pharmacy to provide direct patient care and medication management services. As clinic needs and available budgeted time allow, the CPP may also contribute to select quality‑improvement initiatives, workflow optimization efforts, or practice advancement activities relevant to the assigned clinical area.

The successful candidate may have opportunities to engage in educational mentorship or experiential precepting for pharmacy students and postgraduate pharmacy residents, dependent on clinic assignment, coverage day, and available funded time.

Responsibilities

• Clinical Responsibilities: Build a clinical practice that collaborates with physicians and providers to care for clinic patients; work closely with clinic physicians and providers to determine appropriate treatment plans; provide longitudinal dosing and monitoring of appropriate regimens; align daily practice with medication‑management goals set forth by the institution or pharmacy director; educate patients and families about their treatment plans and subsequent medication management; manage toxicities and adverse effects; manage transitions of care for patients from the acute to ambulatory care setting and vice versa.
• Distributive Responsibilities: Identify and enroll eligible patients in specialty pharmacy services, including UNC SSC Pharmacy, and serve as a contact for the clinic; support retail distributional operations; conduct patient onboarding and follow‑up phone calls to support Pharmacy Adherence Centered Therapy (PACT); coordinate with UNC SSC Pharmacy and the UNC Medication Assistance Program (MAP).
• Precepting/Teaching/Scholarly Activities: Educate UNC Eshelman School of Pharmacy students and UNC Medical Center pharmacy residents on experiential rotations; serve as a standing committee member on select groups of committees within UNC Medical Center and/or Eshelman School of Pharmacy; design and implement projects; conduct research and prepare manuscripts for publication.
• Leadership: Provide instruction to physicians, nurses, ancillaries, residents and/or students; develop programs/lectures; participate in community outreach programs; lead quality improvement initiatives.
• Quality: Engage in continuous quality improvement by identifying and implementing processes for improvement; consult with management for assessment, problem‑solving, decision‑making, and evaluating clinical/system issues and models of care; develop new clinical programs or services within the clinic/department to improve patient care.

Education Requirements

Bachelor's Degree in Pharmacy.

Licensure/Certification Requirements

CPP must be a registered pharmacist and licensed CPP in the state of NC. Credentialing and privileging must be completed through the appropriate UNC HCS accrediting body as an APP prior to start date.

Professional Experience Requirements

• Completion of a PGY2 residency in Ambulatory Care or Pharmacotherapy, or equivalent clinical experience.
• Three to five years of ambulatory care practice experience strongly preferred.
• Board Certification (e.g., BCACP, BCPS) preferred.
