Job summary

Northeast Georgia Health System seeks an Ambulatory Care Pharmacist to lead medication safety initiatives, develop clinical pathways, and coordinate education for providers and staff across NGPG.

The role emphasizes collaboration with providers, care management, and population health teams to optimize patient outcomes and enhance pharmacotherapy across ambulatory settings at NGHS in Georgia.

Qualifications

• Georgia licensed PharmD; must have BCACP/BCPS/BCOP/BCPP/CGP or be eligible for the exam.
• Doctor of Pharmacy degree is required.
• 2–3 years of recent clinical pharmacist experience (hospital or ambulatory care).
• Willingness to work irregular/extra hours and travel in-state as needed.

Responsibilities

• Obtain and analyze data on provider prescribing practices.
• Develop, implement, and manage a NGPG formulary and ambulatory antibiogram.
• Conduct comprehensive medication reviews for polypharmacy and safety.
• Design, implement, and monitor antimicrobial stewardship in ambulatory care.
• Educate patients and caregivers to improve self-management and safety.
• Collaborate with care team to improve patient safety and outcomes.
• Lead pharmacy staff in ambulatory care and oversee a pharmacy residency program.
• Attend and participate in ongoing quality and safety initiatives.
