Job summary

UVA Health Outpatient Retail Pharmacy seeks an Ambulatory Pharmacist to extend patient care beyond dispensing by applying clinical expertise, engaging patients, and collaborating with physicians, nurses, and care teams. You will work across UVA Health's ambulatory network with access to EMR to support medication decisions.

The role rotates through service areas, supports transitions of care, immunization programs, MTM, and vaccination services, while maintaining patient safety and quality care

Qualifications

• Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.
• Experience: PGY‑1 residency or equivalent clinical experience preferred.
• Licensure: Virginia State Board of Pharmacy License or within 60 days of hire. American Heart Association (AHA) Health Care Provider BLS certification required.

Responsibilities

• Reviewing prescriptions for clinical appropriateness, safety, and efficacy.
• Utilizing EMR data including diagnoses, allergies, medication histories, laboratory values, and provider notes to support clinical decision‑making.
• Collaborating directly with physicians and care teams to resolve medication‑related concerns.
• Counseling patients on medications, treatment plans, and disease‑state management.
• Verifying medication products prior to dispensing.
• Identifying and resolving medication access barriers.
• Supporting discharged patients through medication reconciliation and transitions of care.
• Coordinating prescription fulfillment and patient education services.
• Conducting vaccine screenings and administering immunizations in dedicated clinical spaces.
