The role involves providing evidence-based pharmaceutical care in ambulatory settings, designing, implementing, monitoring, and documenting therapeutic plans, and collaborating with providers and interdisciplinary teams to optimize patient outcomes.

Qualifications

• Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
