Job summary

Southern New Hampshire Health in Nashua is seeking an LPN to join ambulatory practice teams delivering high-quality, compassionate care. You will work under RN supervision to support nursing processes, clinical operations, and an exceptional patient experience.

Responsibilities include triage, assessments, medication administration, vaccines, documentation in EMR, and collaboration with physicians and nurses to improve outcomes. Strong communication and organizational skills are essential.

Qualifications

• Graduate of an accredited school of nursing or currently enrolled in an RN program with LPN license.
• Current LPN licensure in NH or NLC state.
• Current BLS/CPR certification.
• Strong nursing principles and patient care practices.
• Experience with electronic medical records and basic office software.

Responsibilities

• Participate in nursing process during in-person visits, telephone triage, and telehealth encounters.
• Assist with patient assessments, interventions, and care plan evaluation.
• Collaborate with physicians and nurses to support coordinated patient care.
• Educate patients on medications, self-management, nutrition, and wellness.
• Document patient care accurately in the electronic medical record.
• Assist providers during examinations, treatments, and diagnostic procedures.
• Administer medications, vaccines, and treatments; perform point-of-care testing; collect specimens.
• Support preventive care and chronic disease management through pre-visit planning and outreach.
• Maintain HIPAA, infection prevention, safety standards, and policies.
• Engage in ongoing education and competency development.
