Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• Bachelor’s Degree in Nursing, preferred.
• Current NYS RN license required; specialized certifications as needed.

Responsibilities

• Conducts comprehensive patient interviews and reviews charts; assesses GI, cardiovascular, respiratory, renal and neurological status; determines mobility, sensory deficits, prostheses use and skin condition.
• Develops patient outcome statements and individualized goals; coordinates cost-effective use of supplies, equipment and medications; documents and communicates plan of care.
• Reassesses patients, revises plans as needed, administers prescribed treatments, and ensures safety and aseptic technique.
• Provides patient and family education; documents outcomes; participates in discharge planning and referrals.
• Demonstrates leadership and clinical judgment; directs/delegates activities on the patient care unit; collaborates with physicians and health team members.
• Maintains accurate documentation of treatments and conditions; communicates changes to care team and appropriate agencies; supports care coordination during emergencies.
• Performs related duties as essential functions of the job; adheres to policies under the Americans with Disabilities Act.
