Qualifications

• Master of Science in Nursing degree required.
• Five years management experience in a healthcare setting required.
• Three years clinical experience preferred.
• Experience in care coordination, care management, population health, or disease management preferred.

Responsibilities

• Directs the management of ambulatory nursing operations and the clinical practice of ambulatory primary, specialty, and urgent care clinics.
• Oversees regulatory and safety activities.
• Develops improvement strategies by identifying and evaluating trends, needs, forecasts and established measurements to provide quality care.
• Provides programmatic leadership for the continuum-based operating model for care management and care coordination services for SMG.
