Qualifications

• Active Wisconsin RN license or multi-state license.
• APN prescriber certification by the Wisconsin State Board.
• Certification by ANCC or AANP for Nurse Practitioners or Physician Assistant license.
• PA-C certification by the NCCPA.
• DEA registration or eligibility to obtain.

Responsibilities

• Assess, diagnose and treat patients in ambulatory care in collaboration with PHMG physician.
• Coordinate care and provide treatment across multiple clinic sites as needed.
