Job summary

ProHealth Care is seeking an Advanced Practice Provider to deliver diagnostic and therapeutic health care in the ambulatory care setting, in collaboration with a designated PHMG physician. The role requires licensure in Wisconsin, APN/PA credentials, and collaboration within a multidisciplinary team.

The position may require travel to multiple clinic sites and a willingness to work within a collaborative practice framework in accordance with state law.

Qualifications

• Master’s degree in Nursing required for Nurse Practitioners.
• Bachelor’s degree for Physician Assistants.
• Two years of clinical experience preferred; new graduates considered if highly qualified.
• Ability to travel to various clinic sites.

Responsibilities

• Provide diagnostic and therapeutic health care in ambulatory care setting in collaboration with a designated PHMG physician.
• Assess, diagnose, and treat patients in ambulatory care.
• Coordinate care with the PHMG team and other healthcare professionals.
