Qualifications

• US citizenship required
• Pharmacy degree from ACPE-accredited school (Pharm.D. or BS Pharmacy)
• Full current license to practice pharmacy in a U.S. state or territory
• English language proficiency required
• Clinical pharmacy specialist responsibilities include initiating/modifying/discontinuing medication therapy and intensive MTM

Responsibilities

• Operate at the highest level of clinical practice within ambulatory care mental health clinics
• Initiate, modify or discontinue medication therapy as a mid-level provider
• Consult with intensive medication therapy management
• Design, implement, assess, monitor and document therapeutic plans
• Perform physical assessments and order laboratory tests as needed
