Job summary

Prime Healthcare is seeking an Ambulatory Medication Access and Referral Coordinator to support the Ambulatory Services Team. You will manage referrals, review patient information, schedule appointments, and ensure accurate EMR records while assessing insurance for medication access.

This role requires expertise in prior authorization, 340B program knowledge, and coordination with providers, pharmacies, payers, and patients to minimize barriers and expedite access to medications in ambulatory

Qualifications

• High school diploma or equivalent required.
• Current State of IL pharmacy technician license in good standing.
• Certified Pharmacy Technician (CPhT) credential.
• Minimum 3 years of experience in ambulatory care, specialty pharmacy, or 340B settings.
• Proficiency in electronic medical record (EMR) systems and insurance authorization platforms.
• Knowledge of HIPAA regulations and patient confidentiality standards.

Responsibilities

• Review and process referrals for ambulatory care services.
• Coordinate patient scheduling and outreach.
• Maintain EMR records of activities and outcomes.
• Ensure proper insurance authorization and 340B program compliance.
• Serve as liaison between providers, pharmacies, payers, and patients to expedite access.
