HealthPartners in Minnesota is seeking an LPN/CMA to support providers and the healthcare team in delivering safe, efficient patient care across multiple clinic areas.

The role involves preparing patients, taking vital signs, documenting in the EHR, assisting with procedures, and ensuring smooth patient flow in a fast-paced setting. Minnesota LPN licensure and current BLS are required.

Qualifications

• Graduate from an accredited school of medical assisting/practical nursing.
• Maintain a current Minnesota practical nurse’s licensure.
• Current BLS provider status per AHA guidelines (online BLS not accepted).
• Proficiency with electronic health record systems.

Responsibilities

• Conduits focused patient assessments in ambulatory clinic setting (intake/rooming, vital signs, health data); report changes to RN/Clinician.
• Implement delegated components of patient plan of care and communicate updates to clinicians.
• Prepare, administer, and document medications and vaccines per clinic protocols; monitor effects and provide instructions.
• Perform ordered clinical procedures and point-of-care activities; maintain aseptic technique; collect/label specimens.
• Act as liaison with patients/families; relay health information and provide education on examinations and home care.
• Document observations and patient outcomes in the EHR (vital signs, meds, procedures, referrals).
• Coordinate care delivery and clinic operations; manage patient flow and transitions of care.
