Qualifications

• Graduate of an accredited registered nursing school.
• Associate's Degree in Nursing or Bachelor’s Degree in Nursing.
• Upon hire: Registered Nurse License.
• Registered Nurse License in state where care will be provided.
• Upon hire: National Provider BLS - American Heart Association.
• 18 months professional nursing experience.

Responsibilities

• Provide comprehensive nursing care in ambulatory care setting.
• Facilitate care coordination using the nursing process (assess, plan, implement, evaluate).
• Deliver care via direct and telephonic/telehealth or electronic encounters.
• Collaborate with the health care team to support clinicians and ensure regulatory compliance.

Education

Associate's Degree in Nursing or Bachelor’s Degree in Nursing

Registered Nurse License (state where care will be provided)

National Provider BLS - American Heart Association
