Qualifications

• Diploma from an accredited school/college of nursing; BSN or ADN required
• BLS provider certification required prior to hire (American Heart Association)
• Current Registered Nurse License State of Illinois
• 1-2 years of clinical experience preferred

Responsibilities

• Patient care and assessment including vital signs and history; develop care plans
• Nurse triage to determine urgency and next steps
• Care coordination and communication with physicians and specialists
• Patient education and support on medications, conditions and lifestyle changes
• Administrative and documentation duties including charting and scheduling
