Qualifications

• Active State Pharmacist License in good standing in the state in which the employee works.
• Board Certification in a Pharmacy Specialty (various BC certifications listed) as applicable to ambulatory care.
• Knowledge of evidence-based clinical pharmacy practice and pharmacotherapies.

Responsibilities

• Prescribe pharmaceutical agents under a collaborative drug therapy management agreement.
• Participate in evaluation and management of diseases in collaboration with other providers.
• Order and interpret drug therapy tests.
• Ensure patients receive safe, efficacious, and cost-effective medication therapy.
• Collaborate to resolve medication access and adherence issues.
• Provide patient/family education on medications and nonpharmacologic management.
