Job summary

CommonSpirit Health is seeking a Clinical Pharmacist Ambulatory Services to deliver high-quality, evidence-based care within a multidisciplinary team and improve patient outcomes.

Based at Centennial, CO in the Mountain Region’s Corporate Service Center, the role emphasizes collaborative drug therapy management, comprehensive clinical assessments, and patient education, with some remote work options.

Qualifications

• Graduation from an accredited school of pharmacy.
• Active Pharmacist License in good standing in the state(s) where employed.
• Knowledge of evidence-based clinical pharmacy practice and pharmacotherapies.
• Knowledge of Medicare, Medicaid and commercial plans.
• Ability to provide care appropriate to all age groups with infection control and safety measures.

Responsibilities

• Prescribe agents as per the collaborative drug therapy management agreement.
• Participate in evaluation and management of diseases in collaboration with providers.
• Ensure access to safe, efficacious, cost-effective medication therapy.
• Address medication-related issues to support care coordination.
• Order and interpret drug-therapy related tests.
• Provide focused teaching to patients and families on medications and non-pharmacologic care.
