Job summary

Mountain Region Support seeks aClinical Pharmacist Ambulatory Services to deliver evidence-based care in a multidisciplinary team, optimizing medication therapy and patient outcomes from its Centennial, CO ambulatory setting.

You will manage complex regimens, perform comprehensive clinical assessments, and act as a subject matter expert for providers and patients, ensuring safe, effective care aligned with regulatory standards.

Qualifications

  • Graduation from an accredited school of pharmacy.
  • Active Pharmacist License in good standing in the state(s) where employed.
  • Knowledge of evidence-based clinical pharmacy practice and pharmacotherapies.
  • Knowledge of Medicare, Medicaid, and commercial plans.
  • Ability to provide care appropriate to all age groups and manage infection control, safety and risk.

Responsibilities

  • Prescribe pharmaceutical agents as outlined in the collaborative drug therapy management agreement, including initiating, adjusting, and discontinuing therapy.
  • Participate in evaluation and management of diseases and health conditions with other providers.
  • Ensure patients have access to safe, efficacious, and cost-effective medications.
  • Address medication-related issues to support care coordination and transitions of care.
  • Order and interpret drug-therapy related tests.
  • Provide patient/family education on medication use and non-pharmacologic management.

Job Summary and Responsibilities

As ourClinical Pharmacist Ambulatory Services, you will deliver high-quality, evidence-based care within a multidisciplinary team to optimize medication therapy and improve patient outcomes. You will play a key role in bridging clinical pharmacy expertise with direct patient support to achieve our organizational quality goals.

Every day you will manage complex medication regimens and perform comprehensive clinical assessments in alignment with institutional protocols and regulatory standards. You will ensure the delivery of efficient, accurate pharmaceutical care while acting as a vital subject matter expert for both healthcare providers and patients.

To be successful in this role, you will hold a current pharmacy license and possess a deep commitment to evidence-based medicine and proactive patient management. We are seeking a collaborative, detail-oriented professional capable of thriving in a fast-paced ambulatory environment to enhance our standard of clinical excellence.

  • Provides patients and/or families with focused teaching on safe and effective use of prescribed medications and non-pharmacologic management of chronic health conditions.
Job Requirements

Required

  • Graduation from an accredited school of pharmacy.
  • Active Pharmacist License in good standing in the state(s) in which the employee works (PHARM:XX).
  • Current knowledge of evidence-based clinical pharmacy practice and pharmacotherapies.
  • Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans.
  • Demonstrated knowledge and skills necessary to provide care appropriate to all age groups of patients served, as well as infection control, safety, and risk management standards as they relate to the ambulatory pharmacy setting.

Preferred

  • Pharm.D. with:
    • Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities, or
    • Completion of a PGY2 Ambulatory Care Pharmacy residency, or
    • Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities.
