Responsibilities

• Partner with physicians and clinic staff to optimize medication therapy.
• Coordinate specialty medication management including access and prior authorizations.
• Monitor laboratory results and medication safety.
• Assess immunization needs and provide patient education.
• Conduct ongoing clinical follow-up with patients in the care team.
• Develop workflows and implement best practices for ambulatory specialty pharmacy.
