Job summary

Geisinger Community Health Services seeks an ambulatory clinical pharmacist embedded in a multidisciplinary team to optimize medication therapy in the ambulatory setting. You will work under a collaborative practice agreement, provide direct patient care by in-person or telephone interventions, and partner with physicians to resolve medication problems and set treatment goals.

Qualifications

• Pharmacist license in PA or eligibility within 90 days.
• Bachelor's degree in Pharmacy is required.
• Minimum 2 years of related work experience.
• Strong communication and leadership skills.

Responsibilities

• Deliver high-quality medication disease management services for the assigned patient care area.
• Demonstrate proficiency in medications, conditions, and current treatment guidelines.
• Gather clinical data to guide decisions on medication therapy and related conditions.
• Assess therapy for appropriateness, effectiveness, safety, and tolerability; adjust regimens as needed.
• Provide patient education and document encounters in the electronic health record (EHR).
• Attend and contribute to department education programs; train staff and students.
• Assist ambulatory leadership in implementing new clinical programs and site-wide initiatives.
• Advise and supervise clinic assistants and interns; ensure accuracy of their work.
• Collaborate with facility staff to integrate amb. pharmacy services into workflow and demonstrate program value.
• Review and revise departmental policies and procedures as requested.
