Job summary

Geisinger is seeking an ambulatory clinical pharmacist to ensure safe, effective, and cost-efficient medication therapy in an ambulatory setting. You will work within a multidisciplinary clinical team under a physician collaborative practice agreement and provide direct patient care via in-person or telephone interventions.

You will review medications for indications, effectiveness, safety, tolerability, and adherence, titrate therapy, and address care gaps to maximize patient outcomes,

Qualifications

• Doctor of Pharmacy degree and licensure to practice pharmacy.
• Licensed Pharmacist (Pennsylvania) within 90 days or as required by state licensure.
• Experience: minimum of 2 years in related work experience.

Responsibilities

• Deliver high-quality medication disease management services within ambulatory care.
• Collaborate with the multidisciplinary team under a physician collaborative practice agreement.
• Assess patient medications for indication, effectiveness, safety, tolerability and adherence.
• Titrate, manage and monitor medication therapy to optimize outcomes.
• Provide education to patients and staff; maintain documentation in the EHR.
• Lead, train and supervise interns, residents and staff as needed.
