Job summary

Geisinger Health seeks an ambulatory clinical pharmacist to provide safe, effective, cost-conscious medication management in an ambulatory setting. You will work with a multidisciplinary team under a collaborative practice agreement, offering direct patient care via in-person or telephone interventions.

You will review medications for indication, efficacy, safety, tolerability, and adherence, titrate therapy, and address gaps in care while educating patients and staff and documenting encounters

Qualifications

• Bachelor's Degree in Pharmacy required.
• Minimum of 2 years related work experience required.
• Licensed Pharmacist in Pennsylvania within 90 days.
• Excellent interpersonal and supervisory skills.

Responsibilities

• Deliver high-quality medication disease management within ambulatory care.
• Work within a multidisciplinary clinical team under a collaborative practice agreement.
• Consult with patients and physicians to optimize medication therapy and goals.
• Titrate, monitor, and adjust therapies; assess indications, effectiveness, safety, tolerability, and adherence.
• Provide education, documentation, and materials to patients and staff; participate in department programs and training.
