Job summary

Baptist Health Arkansas is hiring an Ambulatory Clinical Pharmacist I to support clinics within the Baptist Health system. The role spans remote and face-to-face patient care, focusing on safe, effective medication therapy management in an ambulatory setting.

The pharmacist will collaborate with physicians and multidisciplinary teams, titrate therapy, educate patients, and participate in quality improvement initiatives.

Qualifications

• ACPE-accredited College of Pharmacy with PharmD degree.
• Active Arkansas State Board of Pharmacy license to practice.
• 4 years’ experience as a full-time Pharmacist or 2 years in an Ambulatory Clinical Pharmacist role or completion of ASHP-accredited PGY1 residency.
• Eligible to precept APPE students and PGY1 residents as defined by ASHP standards.
• Active post-licensure certification by Board of Pharmacy Specialties (BPS).
• Experience in direct & indirect patient care initiatives including quality improvement in ambulatory settings.
• Strong verbal and written communication skills.

Responsibilities

• Provide safe, effective, cost-conscious medication therapy for ambulatory patients.
• Consult with patients and providers to resolve medication-related problems and set treatment goals.
• Titrate, manage, and monitor therapy to optimize outcomes; address gaps in care.

Job description

Working Hours: Part-Time 20 hours per week and 40 hours per pay period. Need to POST job code 25029 - Ambulatory Clinical Pharmacist I - position

Summary:

Ambulatory Clinical Pharmacists will practice within the Baptist Health or embedded within a multidisciplinary clinical team to support clinics. The primary responsibility of this pharmacist is to assure the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. The pharmacist in this role will interact with patients and providers remotely and face-to-face as needed. The pharmacist will consult with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals.

The pharmacist assesses each of the patient’s medications to determine that each is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, cost effective and able to be taken by the patient as intended. The pharmacist will titrate/manage/monitor the patient’s medication therapy to optimize clinical outcomes and will identify and address gaps in care. Additional responsibilities include patient and provider education, disease management, comprehensive medication reviews, transitions of care, motivational interviewing, precepting students and residents, and assisting in outpatient medication access and adherence.

Pharmacists must have a patient-centered mindset, excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.

• Graduate of an ACPE accredited College of Pharmacy with a Doctorate of Pharmacy degree
• Active license to practice Pharmacy from the Arkansas State Board of Pharmacy
• 4 years’ experience as a full-time Pharmacist or 2 years experience in an Ambulatory Clinical Pharmacist role or completion of ASHP-accredited PGY1 residency program.
• Eligible to precept APPE students and PGY1 pharmacy residents as defined by latest ASHP standards
• Active post-licensure certification by Board of pharmacy specialties (BPS)
• Demonstrated and documented expertise and experience in direct & indirect patient care initiatives including quality improvement in an ambulatory setting
• Strong verbal and written communication skills

This job will be authorized40.00 hours bi-weekly.
